Distributor: Hind Fashions, of New York, N.Y. Manufactured in: China

Hazard: The jackets have a drawstring through the hood which can pose a strangulation hazard to children. In February 1996, CPSC issued guidelines (which were incorporated into an industry voluntary standard in 1997) to help prevent children from strangling or getting entangled on the neck and waist drawstrings in upper garments, such as jackets and sweatshirts.

Incidents/Injuries: None reported.

Description: This recall involves boys' hooded jackets with drawstrings that are blue suede on the outside and have fur lining on the inside. "Hind Leather" is printed on the tag on the back of the neck. They were sold in sizes 6 through 12. This recall also involves a black quilted leather hooded jacket with drawstrings that has fur surrounding the hood. "Lil' Phat" is printed on the tag on the back of the neck. They were sold in sizes small, medium and large.

Sold exclusively at: Burlington Coat Factory stores nationwide January 2006 through September 2009 for between $30 and $90.

Remedy: Consumers should immediately remove the drawstrings from the garment to eliminate the hazard or return the garment to Hind Fashions or Burlington Coat Factory for a full refund.

Hazard: Strings on the detachable cape can pose a strangulation hazard to young children. Description: This recall involves S. Rothschild girls' wool coats with a detachable cape. The coats were sold in pink, red, blue and vanilla with a faux fur cape and in charcoal and vanilla with a faux fur trimmed cape. Two faux pompoms are attached to the end of strings that hang from the cape. The sewn-in neck tag reads, "ROTHSCHILD SINCE 1881."

Children's Hooded Jackets with Drawstrings Recalled by Lollytogs Due to Strangulation and Entanglement Hazards

Name of product: Rim Rocka Boys' Hooded Jackets and Pelle Pelle Girls' Hooded Jackets

Units: About 23,000 Incidents/Injuries: None reported.

Distributor: Lollytogs Ltd., of New York, N.Y.

Hazard: The jackets have a drawstring through the hood and the vests have a drawstring through the waist which can pose strangulation and entanglement hazards to young children. In February 1996, CPSC issued guidelines (which were incorporated into an industry voluntary standard in 1997) to help prevent children from strangling or getting entangled on the neck and waist drawstrings in upper garments, such as jackets and sweatshirts.

Description: This recall involves boys' Rim Rocka hooded sweatshirts that are black or navy blue with red and white trim or gray and white trim and have a large number embroidered on the front of the sweatshirt. The drawstrings are attached at the hood. They have an emblem on the left shoulder that reads "RR". There is a tag at the back of the neck that reads "Rim Rocka" OFFICIAL SPORTS APPAREL". This recall also involves girls' Pelle Pelle hooded jackets that have a zipper front that zippers to the top of the hood. They come in either pink or white with pictures of bottle caps printed on the hood and sleeves. It reads "Soda Club pelle pelle" on the top right of the sweatshirt. The tag at the back of the neck of the sweatshirt reads "pelle pelle". The drawstrings are attached at the waist. The sweatshirts were sold in sizes 2T-14.

Sold at: Burlington Coat Factory stores and at various small retailers nationwide from February 2008 through September 2009 for about $14.

Manufactured in: Pakistan and Bangladesh

Remedy: Consumers should immediately remove the drawstrings from the garment to eliminate the hazard or return the garment to the place of purchase for a refund or credit.

Hazard: The high powered magnets sets were labeled "Ages 13+" and do not meet the mandatory toy standard F963-08 (effective August 17, 2009) which requires that such powerful magnets are not sold for children under 14. Magnets found by young children can be swallowed or aspirated. If more than one magnet is swallowed, the magnets can attract each other and cause intestinal perforations or blockages, which can be fatal.

Incidents/Injuries: The firm has received two reports of children swallowing one or more magnets. No injuries were reported.

Description: This recall involves the Buckyballs(r) high powered magnets sets labeled "Ages 13+". The set contains 216 powerful rare earth magnets. It is intended to build unlimited shapes and patterns. Since March 2010, Buckyballs(r) high powered magnets sets were labeled "Keep Away From All Children" and are not being recalled.

Sold at: The recalled magnets were sold at a variety of stores, including stores selling children's toys, stationery and office supplies and various online sites from March 2009 through March 2010.

Remedy: Consumers should take the Buckyballs(r) high powered magnets sets labeled "Ages 13+" away from children under 14 immediately and contact Maxfield and Oberton for instructions on receiving a refund upon return of a complete set of magnets.

Infant Death Prompts Recall of Ring Slings Made by Sprout Stuff Due to Suffocation Risk

CPSC and Sprout Stuff are aware of one report of a death of a 10-day-old boy in the recalled sling in Round Rock, Texas in 2007.

The Sprout Stuff infant ring sling is fabric/natural muslin and comes with or without a shoulder pad. The sling is worn by parents and caregivers to carry a child up to two years of age. "Sprout Stuff" is printed on the back side of the tail's hem.

Sprout Stuff sold the recalled infant slings, which were made in the United States, directly to consumers between October 2006 and May 2007 for between $35 and $45. Sprout Stuff is directly contacting known purchasers of the recalled infant slings.

Consumers should immediately stop using the recalled slings and contact Sprout Stuff to return the sling for a full refund. Additional Message from CPSC: On March 12, 2010, CPSC issued a warning about sling carriers for babies. Slings can pose two different types of suffocation hazards to babies. In the first few months of life, babies cannot control their heads because of weak neck muscles. The sling's fabric can press against an infant's nose and mouth, blocking the baby's breathing and rapidly suffocating a baby within a minute or two. Additionally, where a sling keeps the infant in a curled position bending the chin toward the chest, the airways can be restricted, limiting the oxygen supply. The baby will not be able to cry for help and can slowly suffocate. CPSC has determined that a mandatory standard is needed for infant sling carriers. While a mandatory standard is being developed, CPSC staff is working with ASTM International and concerned companies such as Infantino to quickly develop an effective voluntary standard for slings. There currently are no safety standards for infant sling carriers.

McDonald's Recalls Movie Themed Drinking Glasses Due to Potential Cadmium Risk

Name of Product: "Shrek Forever After 3D" Collectable Drinking Glasses

Hazard: The designs on the glasses contain cadmium. Long term exposure to cadmium can cause adverse health effects.

Description: The "Shrek Forever After 3D" collectable drinking glass are 16 ounce glasses that came in four designs, Shrek, Fiona, Puss n' Boots, and Donkey.

Sold exclusively at: McDonald's restaurants nationwide from May 2010 into June 2010 for about $2.

Manufactured in: United States

Remedy: McDonald's is asking consumers to immediately stop using the glass out of an abundance of caution. Visit www.mcdonalds.com for additional instructions on how to obtain a full refund.

Children's Deaths Prompt Recall of Toy Dart Gun Sets Sold Exclusively at Family Dollar Stores

WASHINGTON, D.C. - The U.S. Consumer Product Safety Commission (CPSC), in cooperation with Family Dollar Stores Inc., of Charlotte, N.C., is announcing the voluntary recall of 1.8 million toy dart gun sets, due to an aspiration hazard. CPSC and Family Dollar have received reports of two asphyxiation deaths involving a 9-year-old boy in Chicago, Ill. and a 10-year-old boy in Milwaukee, Wis.

If a child places the soft, pliable plastic toy dart in his/her mouth, the toy can be inhaled into the throat and prevent the child from breathing.

The toy dart guns, imported by Henry Gordy International Inc., of Plainfield, N.J., were sold exclusively at Family Dollar stores. Family Dollar is working cooperatively with CPSC to announce this recall because Henry Gordy International has refused to recall the toy dart gun sets.

The "Auto Fire" toy target set is sold with either an orange and yellow toy gun (item #P238) or a blue and yellow toy gun (item #9328), eight orange darts and a small target with numbers from 1 to 20. The gun's item number is located on one side of the handle in raised lettering. The soft plastic darts measure just over one inch in length and have a small suction cup at one end with a diameter of about ½ inch. The target board is black, yellow, red and green, and measures five inches in diameter. "Gordy Toy" for "Ages 8+" is printed on the packaging.

The toy sets were sold exclusively at Family Dollar stores nationwide from September 2005 through January 2009 for about $1.50.

The products were manufactured in China.

Consumers should immediately take the toy target sets from children and discard them. Consumers can also return the guns and darts to a Family Dollar store for a full refund. Target Recalls Storage Trunks Due to Strangulation Hazard

WASHINGTON, D.C. - The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed. It is illegal to resell or attempt to resell a recalled consumer product.

Name of Product: Woven Storage Trunks Units: About 350,000

Importer: Target Corp., of Minneapolis, Minn.

Hazard: The lid of the trunk can drop suddenly when released, posing a strangulation hazard to small children opening or reaching into the trunks.

Incidents/Injuries: CPSC has received two reports of injuries that occurred when the storage trunks' lids suddenly closed on children, including one report of an 18-month-old girl who reportedly suffered brain damage when the trunk's lid came down on the back of her neck and pinned her throat against the rim of the trunk.

Description: The recall involves 14 different models of the storage trunks made of woven rattan, abaca or banana leaf with standard hinges. They measure more than 1.1 feet in length, width and depth and are brown or natural color.

Sold exclusively at: Target stores nationwide and on the Web at www.target.com from February 2009 through April 2010 for between $50 and $130.

Manufactured in: China and Philippines

Remedy: Consumers should immediately stop using the recalled storage trunks and return them to any Target store for a full refund or replacement product.

C&T International/Sorelle Recalls Cribs Due to Strangulation and Suffocation Hazards

Name of Product: Drop-Side Cribs Units: About 170,000

Distributor: C&T International/Sorelle, of East Rutherford, New Jersey. Before 2003, the company also distributed under the name Golden Baby Inc.

Hazard: The cribs' drop-side hardware can disengage from the tracks, causing the drop side to detach from the crib. When the drop-side partially detaches, it creates space between the drop side and the crib mattress. The bodies of infants and toddlers can become entrapped in the space, which can lead to strangulation and/or suffocation. Complete detachment of the drop sides can lead to falls from the crib. In addition, slats can detach from the sides of the cribs. Infants and toddlers can then become entrapped, strangle or fall out of the crib.

Incidents/Injuries: CPSC and C&T/Sorelle have received reports of 104 incidents of drop-side and slat detachments in C&T/Sorelle drop-side cribs. Six infants received bruises and abrasions to the head, face, torso or leg from becoming entrapped or falling after the drop side collapsed. An additional five infants who were entrapped or fell were found by their parents and were not injured.

Description: This recall involves C&T International, Sorelle and Golden Baby wood cribs. The full-size cribs were sold in natural, white and cherry finishes. The model number and/or model name is printed on a label affixed to the footboard or headboard under the Caution statement.

Sold at: Children's product stores and other retailers nationwide from January 2000 through March 2010 for between $300 and $600.

Manufactured in: Italy, Latvia, Brazil, China and Vietnam

Remedy: CPSC urges parents and caregivers to immediately stop using the recalled cribs and find an alternative, safe sleeping environment for their baby. Consumers should contact C&T/Sorelle to receive a free replacement kit. For four of the older models of the cribs - the Hampton, Jackie, Nina and Rita models -- the firm may be unable to supply a part to repair the crib and will, instead, provide consumers with a $100 voucher towards the purchase of another C&T product.

Important Message from CPSC: CPSC reminds parents not to use any crib with missing, broken, or loose parts. Make sure to tighten hardware from time to time to keep the crib sturdy. When using a drop-side crib, parents should check to make sure the drop side or any other moving part operates smoothly. Always check all sides and corners of the crib for disengagement. Disengagements can create a gap and entrap a child. In addition, do not try to repair any side of the crib. Babies have died in cribs where repairs were attempted by caregivers.

Hazard: The garments have a drawstring through the hood which can pose a strangulation hazard to children. Description: This recall involves girls' hooded sleeveless vests with drawstrings sold in sizes 2 through 10. The vests were sold in two colors: blue with pink flowers and black with white polka dots. Both styles have a fake fur trim around the hood and a black sweater-like knit trim at the armholes and waist. The vests have an interior zipper covered by an exterior flap closure and a sewn-in neck tag that reads "mim-pi."

Sold at: Children's clothing stores nationwide from January 2009 through December 2009 for about $70.

Remedy: Consumers should immediately remove the drawstrings from the recalled garments to eliminate the hazard. Consumers can also return the vests to the place of purchase for a store credit.

CPSC Issues Warning on Drop-Side Cribs; 32 Fatalities in Drop-Side Cribs in Last 9 Years

WASHINGTON, D.C. - As part of its commitment to ensure safe sleep for young children, the U.S. Consumer Product Safety Commission (CPSC) is once again warning parents and caregivers about deadly hazards with drop-side cribs. In the last five years, CPSC has announced 11 recalls involving more than 7 million drop-side cribs due to suffocation and strangulation hazards created by the drop side. CPSC staff is actively investigating several other crib manufacturers for potential drop-side hazards as part of a larger effort by the agency to rid the marketplace and homes of unsafe cribs. CPSC will continue to take aggressive action to address any risks and will keep the public informed.

CPSC staff has completed a comprehensive review of crib-related infant fatalities reported to the agency between January 2000 and the present. CPSC staff is aware of 32 infant and toddler suffocation and strangulation deaths and hundreds of incidents that were caused by or related to drop-side detachments in cribs made by various manufacturers.

In addition to the 32 deaths the CPSC staff associated with the drop-side detachments, CPSC has received an additional 14 reports of infant fatalities due to entrapment in cribs that could be related to a drop side. The information obtained was insufficient for staff to conclusively determine whether or not the drop side was involved. Of the 32 deaths that were analyzed, some occurred in cribs where the drop side detached without caregivers noticing the detachment, while some other deaths occurred after a consumer tried to repair the detached drop side, but the repair ultimately failed.

In other incidents, consumers unknowingly installed the drop side or drop-side hardware incorrectly. In several cases, this occurred due to incorrect or confusing directions. In these incidents, the drop side still appeared to function as intended, but the stress on the crib hardware resulted in the drop-side detachment.

CPSC Chairman Inez Tenenbaum has committed to parents and caregivers that there will be a new and vastly improved mandatory federal standard for cribs this year. The standard will incorporate, at minimum, the new voluntary standard banning drop-side cribs from the United States market. Due to the new voluntary industry standard, many manufacturers have already stopped selling drop-side cribs or will do so beginning June 1, 2010.

Drop-Side Hazards

CPSC technical staff has determined drop-side cribs generally have a tendency to be less structurally sound than cribs with four fixed sides. Drop-side hardware is prone to break, deform or experience other problems during normal or foreseeable use. The older the crib, the more problems can be expected. When drop-side hardware breaks or deforms, the drop side can detach in one or more corners from the crib. If an infant or toddler rolls or moves into the space created by a partially detached drop side, the child can become entrapped or wedged between the crib mattress and the drop side and suffocate. Infants can also strangle in the "V" shape formed by a drop side that detaches in an upper corner.

Advice to Consumers

Check your crib regularly and make sure it has not already been recalled.

While CPSC staff cannot say that every drop-side crib is hazardous, based on investigations of incidents we have received, the agency believes that overall most drop-side cribs are more prone to mechanical failure than similar designed fixed-side cribs. In addition, older cribs may not meet current voluntary standards. Factors that contribute to safety problems in older cribs include:

The longer a crib is used, the more wear and tear on hardware and joints, allowing screws to loosen and fall out and plastic parts to flex and break.

Repeated assembly and disassembly increases likelihood that crib parts can be damaged or lost.

Wood warps and shrinks over time and glue can become brittle. This can lead to joint and slat failures.

Important Message from CPSC: CPSC reminds parents not to use any crib with missing, broken, or loose parts. Make sure to tighten hardware from time to time to keep the crib sturdy. When using a drop-side crib, parents should check to make sure the drop side or any other moving part operates smoothly. Always check all sides and corners of the crib for disengagement. Disengagements can create a gap and entrap a child. In addition, do not try to repair any side of the crib. Babies have died in cribs where repairs were attempted by caregivers.

If your baby is less than six months old and is not yet able to push up to his/her hands and knees, you can put your baby to sleep in a bassinet. Make sure your bassinet has not been recalled. Also, you can use a play yard. Do not put additional bedding such as pillows, thick quilts, comforters or anything plush into your baby's sleeping space. More babies die every year from suffocation in plush sleeping environments than from defective cribs.

Gund Recalls to Replace Baby Books Due to Choking Hazard

Name of Product: Gund Baby Paperboard Books

Hazard: The styrofoam used to fill the book binding can detach, posing a choking/aspiration hazard to infants and young children.

Incidents/Injuries: Gund has received three reports of children mouthing the styrofoam that detached from the books. No injuries have been reported.

Description: This recall involves three miniature children's paperboard books with plastic handles designed as baby rattles. They include "Animals" (item number 059174), "Numbers" (item number 059175) and "Colors" (item number 059176). The books were also sold as a three-book set (item number 059173). The name "Gund" and the item number are located on the back of the books. The books measure 4 1/2 inches by 7 inches.

Sold at: Gift and specialty stores nationwide from January 2009 through March 2010 for about $8 and $20 for the set.

Generation 2 Worldwide and "ChildESIGNS" Drop Side Crib Brands Recalled; Three Infant Deaths Reported

WASHINGTON, D.C. - The U.S. Consumer Product Safety Commission (CPSC) is announcing the recall of all Generation 2 Worldwide and "ChildESIGNS" drop side cribs. CPSC is warning parents and caregivers who own these drop side cribs that infants and toddlers are at risk of serious injury or death due to strangulation and suffocation hazards presented by the cribs. CPSC staff urges parents and caregivers to stop using these cribs immediately and find an alternative, safe sleeping environment for their baby. Do not attempt to fix these cribs.

The crib's plastic hardware can break which can cause the drop side of the crib to detach from a corner of the crib. When the drop side detaches, it creates a space into which an infant or toddler can roll and become wedged or entrapped. When a child is entrapped between the drop side and the crib mattress, it creates a risk of suffocation or strangulation. In addition, the crib's mattress support can detach from the crib frame, creating a hazardous space in which an infant or toddler could become entrapped and suffocate or strangle.

CPSC has received reports of three infants who suffocated when they became entrapped between the crib mattress and the drop side when the drop side detached. In July 2007, an eight month old child from Newark, Ohio suffocated when he became entrapped between the drop side and the crib mattress. The drop side of his crib had detached due to a broken plastic stop tab on the lower track. In October 2003, an eight month old child from Richmond, Ind. suffocated when he became entrapped between the drop side and the crib mattress. The plastic hardware on the drop side was broken and allowed the drop side to detach from the crib headboard in one corner. In September 2002, a six month old from Staunton, Va. suffocated when he became entrapped between the drop side and crib mattress. The lower drop side track was missing two screws which allowed it to pull away from the headboard post and detach.

CPSC has also received reports of 20 other drop side incidents, 12 of which involved the drop side detaching in a corner of the crib. In two of these incidents, a child became entrapped. One child suffered bruising from the entrapment. There are five reports of children falling out of the cribs due to drop side detachment. One child suffered a broken arm as a result of the fall.

In addition, CPSC has received 8 reports of mattress support detachment in these cribs. Due to the space created by the detachment, three children became entrapped between the crib frame and the sagging mattress and four children crawled out of the crib. There was one report of cuts and bruises.

Due to the fact that Generation 2 went out of business in 2005, CPSC has limited information about the cribs. Although CPSC does not know the total number of units distributed or the years of production, it is believed that there were more than 500,000 of these cribs sold to consumers. Some of the known model numbers are: 10-110X, 10-210X, 21-110X, 20-710X, 64-315X, 26-110X, 90-257X, 20-810X, 46-715X, 64-311X, 74-315X, 21-815X, 21-810X, 20815X, 308154 and 54915. (The "X" denotes where an additional and varying number may appear at the end of the model number.) However, all Generation 2 Worldwide and "ChildESIGNS" drop side cribs are included in this recall, including those with other model numbers.

The name "Generation 2 Worldwide" appears on a label affixed to the crib's headboard or footboard. Some labels identify the place of manufacture as Dothan, Ala. Others identify China as the country of manufacture. The name "ChildESIGNS" appears on the teething rail of some of the cribs.

The recalled cribs were sold at numerous local furniture and retail stores including Buy Buy Baby, and Kmart and Walmart stores nationwide for between $60 and $160. Consumers should contact the store from which they purchased the crib for remedy information, which will vary between a refund, replacement crib or store credit, depending on the retailer. Consumers are urged to contact CPSC and report any difficulties in obtaining a remedy from their place of purchase.

Important Message from CPSC:

CPSC would like to remind parents not to use any crib with missing, broken, or loose parts. Make sure to tighten hardware from time to time to keep the crib sturdy. When using a drop-side crib, parents should check to make sure the drop-side or any other moving part operates smoothly. Always check all sides and corners of the crib for disengagement. Any disengagement can create a gap and entrap a child. In addition, do not try to repair any side of the crib, especially with tape, wire or rope.

Name of Product: Discovery Kids(tm) Animated Marine and Safari Lamps

Units: About 360,000 Importer: Innovage LLC, of Foothill Ranch, Calif.

Hazard: A defect in the lamp's printed circuit board can cause an electrical short, posing a fire and burn hazard to consumers.

Incidents/Injuries: Innovage has received nine reports of incidents, including seven reports of lamps catching fire, one involving smoke inhalation injury to a child and three involving minor property damage.

Description: This recall involves the Discovery Kids(tm) Animated Marine Lamp with model number 1627121 or 1628626 and the Animated Safari Lamp with model number 1627124 or 1628626.All models have batch numbers beginning with "2". The decorative lamps are silver in color and feature rotating films with marine or safari scenes. "Discovery Kids" is printed on the front top left corner. The batch number is an 11 digit number located on the bottom of each unit. The model number can be found on the bottom of the packaging.

Sold at: Mass merchandisers, department, drug and hardware stores nationwide, online and through direct sales from July 2009 through January 2010 for about $10.

Clarks Children's Shoes Recalled
by C & J Clark America Due to Choking Hazard

WASHINGTON, D.C. - The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed.

Name of Product: Children's Shoes

Units: About 2,000

Distributor: C & J Clark America Inc.,
d/b/a the Clarks Companies N.A., of Newton, Mass.

Hazard: Molded rubber pieces on the sole of the recalled shoes can detach, posing a choking hazard to infants and young children.

Incidents/Injuries: No incidents have been reported in the United States. In the United Kingdom there have been six reports of the rubber pieces detaching, including two reports of children mouthing the rubber piece. No injuries have been reported.

Description: This recall involves Clarks(r) children's shoes sold under the "crawlers" and "hazy daze" style names. The crawlers were sold in infant sizes 2 to 3 1/2; and the hazy daze in sizes 4 to 6 1/2. "Clarks" is printed on the sole underneath the heel and the words "Clarks First Shoes" is printed on a multi-colored label inside the shoe's heel area. Hazard: The recalled workshop sets and trucks have oversized, plastic toy nails that can pose a choking hazard to young children.

Incidents/Injuries: The firm has received one report of an 11-month old boy from Goose Creek, S.C. who choked when the toy nail become forcefully lodged in his throat. The child was hospitalized and made a full recovery.

Description: The oversized, plastic toy nail is about 3 ¼ inches long by 1 ¼ inch in diameter and comes in red or blue colors. It has a large round head; about 1 ¼ inches below the head is a plastic ridge, slightly smaller than the nail head and about 1 inch in diameter. Hazard: The cotton hoodie has a waist drawstring that could pose an entrapment hazard to children. In February 1996, CPSC issued guidelines (pdf) to help prevent children from strangling or getting entangled on the neck and waist drawstrings in upper garments, such as jackets or sweatshirts.

Incidents/Injuries: No injuries have been reported.

Description: The cotton hoodie, which zips in front and has a drawstring at the waist, came in gray with blue lining and cherry with lavender lining. They were sold in girls' sizes SML-XL. ROXY is printed on the front of the gray hoodie and on the neck label on both colors. The style number, B487857, is printed on the inside care label near the waist.

Sold at: Nordstrom, Macy's, Quiksilver Stores and various surf, sport, and other retailers nationwide from April 2009 through August 2009 for approximately $40.

Pirate Expedition figures are 6.5" in height and have assorted accessories including a parrot or a treasure chest.

Burley Design Recalls Child Trailers Due to Risk of Injury

Name of Products: 2009 d'lite ST and Solo ST Child Trailers Units: About 2,700

Manufacturer: Burley Design LLC, of Eugene, Ore.

Hazard: The axle assembly's internal sleeve can loosen, causing one wheel to separate from the trailer. This poses a risk of injury to the child occupant or bike rider.

Incidents/Injuries: No incidents or injuries have been reported. Manufactured in: Philippines

Description: The recalled products are two-wheeled child trailers designed to carry one or two children behind a bicycle. Only the 2009 d'lite ST and 2009 Solo ST trailers with serial numbers that begin with D939 or D948 are included in the recall. The serial number is located in the rear cargo area behind the seat on the lower left rear frame tube. The trailers have "d'lite ST" or "solo ST" screen printed on the cover.

Sold by: Independent bicycle retailers and specialty outdoor retailers nationwide and on various web sites from November 2008 through June 2009 for between $550 and $600.

Remedy: Consumers should stop using the trailer immediately and contact Burley to receive a free repair kit or to locate a retailer to assist with the free repair.

Strangulation Deaths of Children Prompt Recall
to Repair of Roll-Up Blinds and Roman Shades by Lewis Hyman Inc.

Name of Product: ¼" Oval Roll-up Blinds and Woolrich Roman Shades

Units: About 4.2 million roll-up blinds and 600,000 Roman shades

Importer: Lewis Hyman Inc., of Carson, Calif.

Hazard: Roll-up Blinds: Strangulations can occur if the lifting loops slide off the side of the blind and a child's neck becomes entangled on the free-standing loop or if a child places his/her neck between the lifting loop and the roll-up blind material.

Roman Shades: Strangulations can occur when a child places his/her neck between the exposed inner cord and the fabric on the backside of the blind or when a child pulls the cord out and wraps it around his/her neck.

Incidents/Injuries: In November 2007, a 1-year-old boy from Norridgewock, Maine became entangled and strangled in the lift cord loop of a roll-up blind that had fallen into his portable crib. In October 2008, a 13-month-old boy from Conway, Ark. was found with his head between the exposed inner cord and the cloth on the backside of a Roman shade. The cord was not looped around the boy's neck but rather ran from ear to ear and strangled the child.

Description: This recall involves roll-up blinds without release clips (see picture below) and all Woolrich Roman shades. The roll-up blinds have plastic oval-shaped slats that measure about ¼ inch tall. The blinds measure either 72" or 96" long. The bottom rail has a WARNING label advising that "Young children can become entangled and strangle in cord or bead loops" and a label that reads "Lewis Hyman, Inc." and the year of manufacture. Roll-up blinds that have release clips right below the head rail on the backside of the blind are not included in this recall.

The Woolrich Roman shades come in twill fabric and micro-suede fabric and measure 72" long. The head rail has two labels that read "Lewis Hyman, Inc., www.lewishymaninc.com" and "LHI, 005301, Made in China" respectively.

Sold at: The Roman shades were sold exclusively at Target stores nationwide and on Target.com from March 2006 through December 2008 for between $25 and $43. The roll-up blinds were sold at retail stores nationwide from January 1999 through December 2003 for between $6 and $20.

Remedy: Consumers should immediately check the backside of the roll-up blinds to determine if they have release clips. If the roll-up blind does not have release clips, stop using it immediately and contact Lewis Hyman for a free repair kit.

The Roman blind has a continuous looped bead chain that when not attached to the wall or floor, hangs loosely by the blind, posing a strangulation hazard to children.

Incidents/Injuries: CPSC and IKEA received a report of a 2-year-old boy who suffered a near strangulation. His mother found him hanging from the looped bead chain.

Description: This recall includes all sizes/colors of MELINA Roman blinds. These blinds have a sewn-in label at the top edge of the blind with the IKEA logotype, article name (MELINA), 5-digit supplier number 19395, four digit date stamp (YYWW) and the words "Made in Taiwan". On the bottom edge of the blind there is a sewn-in orange/white safety warning label. The blinds are made from 100% polyester.

Sold at: IKEA stores nationwide from August 2006 through June 2008 for between $20 and $40.

Near Strangulation of Children Prompts Recall

Incidents/Injuries: There have been six reports of children becoming entangled in the exposed inner cord on the back of the shade between March 2006 and July 2008. Four children were found with cords entangled around their necks. Scissors were used to release two of these children. Two children were found with red marks around their necks after having freed themselves. No permanent injuries were sustained.

Description: This recall involves all sizes and colors of Pottery Barn Kids roman shades with exposed inner cords on the back. Two sewn-in labels are located at the bottom edge of the shade. The smaller label identifies the manufacturer as "pottery barn kids" while the second larger orange, black and white label warns that "Cords and bead chains can loop around child's neck and STRANGLE." Roman shades with a fabric covering over the cords are not being recalled.

The widths of the Matchstick roman shades are 23, 27, 31, 35 and 36 inches and are brown.

Description: This recall involves custom-ordered made-to-measure manual roller shades. The shades were sold in a variety of colors, fabrics, and sizes. They have a label on an adhesive strip that connects the shade fabric to the roller tube that reads "Lutron Shading Solutions" and "800-446-1503." The shade needs to be removed from the brackets and unrolled completely to access the label. The shade was sold with an orange, black and white hang tag warning consumers that "young children can STRANGLE in cord and bead chain loops." However, the hang tag may have been discarded upon installation. Nearly all shades were professionally installed.

Sold at: Specialty dealers and Expo Design Centers nationwide from January 2000 through April 2009 for between $200 and $2000 per shade.

Hazard: Horizontal Blinds: The blinds do not have inner cord stop devices to prevent the accessible inner cords from being pulled out. If an inner cord is pulled out, a child can become entangled in the loop and strangle.

Vertical Blinds: Strangulations can occur if a child's neck becomes entangled on the free-standing loop formed by the bead chain or by the cord with a weighted device.

Cellular Shades: Strangulations can occur if a child's neck becomes entangled on the shade's free-standing looped cord.

Incidents/Injuries: In May 2006, CPSC received a report of a 4-year-old girl from Pensacola, Fla., who strangled in the loop of a vertical blind cord that was not attached to the wall or floor.

Description: This recall involves all Vertical Land custom-made vertical and cellular window coverings that do not have a cord tensioning device and horizontal blinds that do not have inner cord stop devices. A sticker identification label can be found on the head rail of some of the window coverings which lists the company's name, address and telephone number.

Sold at: Vertical Land stores in Panama City and Pensacola, Fla. from January 1992 through December 2006 for between $60 and $200 (depending on the size of the blind).

Hazard: The shoe has decorative wheels that can detach, posing a choking hazard to young children.

Description: This recall involves the Buster Brown & Co. clog children's shoes with model or retail numbers listed below. These shoes are a plastic molded EVA "water" clog and are designed to resemble a car. The shoes are red, brown, blue, yellow, and pink and most shoes bear the words "CARS," "Transformers Animated," or "Barbie." Simplicity Drop Side Cribs Recalled

Product: Simplicity Drop Side Cribs
Units: About 400,000 Manufactured in: China

Importer/Distributor: Simplicity Inc. and SFCA Inc. of Reading, Pa.

Hazard: The crib's plastic hardware can break or deform, causing the drop side to detach. When the drop side detaches, it creates space between the drop side and the crib mattress. Infants and toddlers can roll into this space and become entrapped which can lead to suffocation. Incidents/Injuries: CPSC is aware of one death involving an 8-month-old child from Houston, Texas who became entrapped and suffocated between the drop side and the crib mattress when a plastic connector on the drop side broke. The child's death was previously reported by CPSC.

CPSC is also aware of an additional 25 incidents involving the drop side detaching from the crib. In six of these incidents, the drop side detached because the plastic flexible tab deformed or broke. In four of the drop side detachment incidents, other plastic parts, including connectors or tracks, deformed or broke. In two of the incidents, two children became entrapped between the drop side and the crib mattress. There were no reported injuries. Description: This recall involves all drop side cribs with a different or "newer" style of plastic hardware from those cribs recalled in September 2007. This newer style of Simplicity hardware can be identified by a flexible plastic tab at the top of the lower tracks. The recalled model numbers include but may not be limited to: 8050, 8325, 8620, 8745, 8748, 8755, 8756, 8765, 8778, 8810, and 8994, 8995, 8996. Sold at: Department stores, children's stores, and mass merchandisers nationwide from January 2005 through June 2009 for between $150 and $300. Remedy: Consumers should immediately stop using the recalled cribs and find an alternative, safe sleeping environment for their baby. Consumers should immediately return the crib to the place of purchase for a refund, replacement or store credit.

Due to Drowning Hazard

Hazard: The leg straps in the seat of the float can tear, causing children to unexpectedly fall into or under the water, posing a risk of drowning. Incidents/Injuries: There have been 31 reports of float seats tearing, causing children to fall into or under the water. No injuries have been reported.

Description: The recalled inflatable baby floats were sold in a variety of styles and colors. The names and model numbers are located on the packaging. A date between 08/02 and 07/08 is molded on the valves of recalled products. Products not included in the recall can be identified with the lettering "NP" molded on the valve. Pacifiers Recalled by Gromex Due to Choking Hazard

Name of Product: Jaloma Pacifiers Units: About 700 Manufactured in: Mexico
Importer: Gromex Inc., of Passaic, N.J.

Hazard: The pacifier mouth guard and the ventilation holes are too small and fail to meet federal safety standards. The pacifier could pose a choking and aspiration hazard to young children.

Incidents/Injuries: None reported.

Description: This pacifier has a ring-shaped handle and a round-shaped mouth guard with two ventilation holes. "Jaloma" is printed on the handle side of the mouth guard and the nipple has a gel-like liquid inside.

Sold at: Retail stores in New Jersey and New York from February 2008 through March 2009 for about $1.

Remedy: Consumers should immediately take the recalled pacifiers away from children and contact Gromex for a refund or exchange.

Kolcraft Recalls 1 Million Play Yards Due to Fall Hazard

Hazard: The play yard's side rail can fail to latch properly and when a child pushes against the rail it can unlatch unexpectedly, posing a fall hazard to children. Incidents/Injuries: The firm has received 347 reports of sides of the play yard collapsing unexpectedly, resulting in 21 injuries to young children, including bumps, scrapes, bruises and one concussion.

Hazard: The hooded sweatshirts have a drawstring through the hood which can pose a strangulation hazard to children. In February 1996, CPSC issued guidelines to help prevent children from strangling or getting entangled on the neck and waist drawstrings in upper garments, such as jackets or sweatshirts.

Incidents/Injuries: None reported.

Description: The boys' hooded sweatshirts have the brand name Ocean Current and RN#67225 number in the neck label. The garments were sold in the color smoke and in sizes S-XL. Sold at: Macy's department stores in Southern California from December 2008 through April 2009 for about $35. Remedy: Consumers should immediately remove the drawstrings from the sweatshirts to eliminate the hazard, or return the garment to either the place of purchase or to C-Mrk for a full refund.

Due To Entrapment and Strangulation Hazards

WASHINGTON, D.C. - The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed. Name of Product: Bonavita "Cabana" Drop Side Cribs Units: 2,000 Importer: LaJobi Inc., Cranbury, N.J., Manufactured in: Vietnam Hazard: The wooden crib slats can detach or break creating a gap, which can pose an entrapment and strangulation hazard to infants and toddlers. Incidents/Injuries: LaJobi has received 16 incident reports of crib slat failures, including 10 reports of slats falling out of rails and 6 reports of slats breaking. No injuries have been reported. Description: This recall involves Bonavita "Cabana" wooden drop side cribs with metal mattress supports. The cribs were manufactured from January 2006 through October 2008 and have production dates between 01 JAN 2006 and 31 OCT 2008. The model name, color, product order (PO) number, production date and other crib identifiers are printed on a label located on the inside bottom rail of the headboard or footboard. The cribs were sold in three finishes, antique white, classic cherry and montego brown. Cribs with other production dates are not affected by this recall. Sold at: USA Baby, Beautiful Beginnings, Buy Buy Baby and other specialty stores nationwide from January 2006 through May 2009 for about $450. Remedy: CPSC urges parents and caregivers to immediately stop using the recalled cribs and find an alternative, safe sleeping environment for their baby. LaJobi Recalls Babi Italia Pinehurst
and Bonavita Hudson Drop Side Cribs
Risk of Entrapment and Suffocation

WASHINGTON, D.C. - The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed. Name of Product: Bonavita "Hudson" and Babi Italia "Pinehurst" Drop Side Cribs Units: About 2,900 Importer: LaJobi Inc., of Cranbury, N.J., Manufactured in: China Hazard: The lower spring pins on the footboard and headboard can pop out of the tracks located on the drop side causing the drop side to detach from the crib. When the drop side detaches, it creates a hazardous gap between the drop side and the crib mattress in which infants and toddlers can become entrapped and suffocate or fall from cribs. Incidents/Injuries: CPSC and LaJobi have received a total of 33 reports of drop side detachment due to spring pin failure. In these reports, two children became entrapped and one child fell from cribs. Both entrapped children were freed by caregivers, one child was bruised. Description: The recalled cribs are wood with a metal mattress support and have drop sides and were sold in a variety of finishes including cherry, espresso, natural, and white. Hudson and Pinehurst drop side cribs with production dates from December 2006 through August 2007 are included in the recall. The recalled cribs have the production date, product order (PO) number, item number, and color printed on a label attached to the footboard or headboard. Sold at: Babi Italia Pinehurst drop side cribs were sold exclusively by Babies "R" Us. Bonavita Hudson drop side cribs were sold at Baby Basics, Beautiful Beginnings, and Buy Buy Baby stores and children's product stores nationwide for about $300. Cribs were sold from December 2006 through December 2007. Remedy: CPSC urges parents and caregivers to immediately stop using the recalled cribs and find an alternative, safe sleeping environment for their baby. LaJobi will provide professional technicians to perform in-home installations of a free retrofit hardware kit. Consumers should contact LaJobi to register for the free retrofit kit service.
